WHAT IS PDAO?
The Persons with Disability Affairs Office (PDAO), under the direct supervision of the Office of the City Mayor, formulates policies and programs for the welfare of Persons with Disabilities (PWDs) and ensures the registration of PWDs in the San Miguel bulacan Registry of Persons with Disabilities.

No. Successful online application does not guarantee ID issuance. The Persons with Disability ID issuance shall officially be accomplished in the nearest Municipal/City Social Welfare Development Office with your complete documentary requirements.
His/Her Guardian should bring an authorization letter and other requirements on behalf of the Persons with Disability.
There are two ways for applying for issuance of Persons with Disability ID thru: (1) walk-in application and (2) online application.
